Please note the following Financial Policies:
Consultation Payments: The consultation fee ensures your protected block of consultation time with Dr. Lewen. The consultation fee is a non-refundable scheduling deposit that is charged immediately upon scheduling your appointment. This fee is due and payable regardless if you elect to fulfill your scheduled appointment and see Dr. Lewen for consultation, and regardless if any treatment is rendered for any reason.
Deposits AND/OR Payments: Should you need to reschedule your procedure, you must give FOURTEEN (14) days prior notice to apply any prepaid fees or deposits to a new date. All prepaid fees and deposits are forfeited if not rescheduled within SIX (6) months of the original procedure date.
